Player's date of birth is required on all USGA entry forms. All online entry applications for players who are minors, upon the date of submission, in the state in which they reside (e.g., New Jersey, under the age of 18) must be submitted by a parent/guardian, and list the parent/guardian's name, phone number and e-mail address.
